Biography of the Author
In addition to more than 2 million views for his more than 200 TV interviews on COVID-19, Dr. Mahmoud Halablab has completed his Ph.D. in Microbiology at
King's College London, the University of London
in 1991, specializing in clinical and medical microbiology.
Dr. Halablab was appointed a university professor at King’s College and spent more than 15 years as an academic before joining Rafik Hariri University in 2009 as a full-time university professor in the College of Arts and Sciences.
He published more than 50 peer-reviewed research papers, a textbook entitled “Infection and Immunity”, and textbook chapters. Some of his research articles have been published in highly prestigious academic journals, including
The Lancet
, one of the world's top medical journal and another article in
Emerging Infectious Diseases Journal
published by CDC. He named
three new bacteria
that have formally been accepted by the
International Committee on Systematics of Prokaryotes.
He is also a regular referee for research articles published in the
Society for Applied Microbiology
and other scientific journals. On several occasions, he was a keynote speaker on water and food-related issues. He is also a regular speaker at several national and international conferences on
Legionella
infections and foodborne diseases.

Newly emerging science named “Sociovirology”; it is a language used by microbes like bacteria and recently by viruses to communicate with each other like packs of wolves. Coronaviruses may have such social life, and if we understand their language, we can interfere with it and treat the diseases they cause.

A question that still needs to be answered for the current Coronavirus pandemic: How many viruses are required (infective dose) to cause COVID-19 disease in humans? If the number of viruses increases would clinical symptoms do too?
سؤال لا إجابة له بعد عن فيروس كورونا! ما هو عدد الفيروسات الذي نحتاجه لإحداث مرض كوڤيد-١٩عند الانسان؟ هل كل ما زاد العدد تزدادالعوارض السريرية؟
Currently, to control the COVID-19 pandemic, a safe and efficient vaccine that induces long-lasting immunity is vital. If this vaccine becomes available, we need doses enough for about 70% of the world population, which means around 6 billion people. From the available advanced vaccines, it is likely to need two doses per person. Therefore, 12 billion doses are required yearly to combat the virus. It’s a long way to go!
حالياً، ليس هنالك حل لجائحة كورونا إلا بوجود لقاح آمن وفعّال ويُحدث مناعة مكتسبة طويلة الأمد. وإن وجد اليوم نحتاج جرعات لتكفي حوالي %70 من سكان الكرة الأرضية أي لحوالي 6 مليار شخص للقضاء على الفيروس. في معظم اللقاحات المتطورة اللآن, كل شخص يحتاج الى جرعتين. يعني نحن بحاجة الى 12 مليار جرعة سنوياً. الطريق طويل !
Good news for women! Men are more likely to die from COVID-19 compared to women. Although they get infected at the same rate, men seem to suffer more severe symptoms than women. This is perhaps since women have high levels of estrogen and progesterone, which help the immune system. Moreover, a significant protein (TLR7) is present twice as much as in men. This protein could recognize and attack the Covid-19 virus.
أخبار سارة للسيدات! نسب الوفيات بسبب كورونا بين الرجال أكثر منها عند النساء. بالرغم من أن نسب الإصابة بكورونا بين الرجال والنساء متساوية، إلا أن بعد الإصابة نرى أن نسب الحالات الخطرة بين الرجال أكثر من النساء، هذا الأمر يعود لعدة أسباب، منها: أن النساء لديهن مناعة أكثر لوجود هرمونات “أستروجين” و”بروجسترون” التي تساعد جهاز المناعة أكثر، وهناك أيضا"بروتين مهم جدا عند النساء وهو (TLR7) وهو متواجد لديهن بضعف الكمية عند الرجال، وهذا البروتين يتعرف ويتصدى لفيروس “كوفيد-19″.
Coronavirus can survive on human skin for more than 9 hours. Washing it with alcohol kills the virus in 15 seconds. Therefore, washing hands, social distancing, and taking the influenza vaccine are urgent matters to reduce the spread of Covid-19. Also, remember Corona and

viruses can infect humans at the same time. Even if the influenza virus infects a person alone, it may weaken his/her immune system and becomes more susceptible to coronavirus.
فيروس كورونا يعيش لأكثر من ٩ ساعات (فيروس الإنفلونزا ساعتين) على جلد الإنسان وغسله بالكحول يقضي على الفيروس خلال ١٥ ثانية. إذن غسل اليدين، التباعد الاجتماعي وأخذ لقاح الانفلونزا هذا الشتاء حاجة ضرورية ملحة للحد من انتشار فيروس كورونا. لا ننسى أن كل من فيروس كورونا والانفلونزا قد يصيبا الانسان في نفس الوقت. وان أصيب الشخص بفيروس الأنفلونزا لوحده, قد يضعف جهاز المناعة لديه ويصبح عرضة لفيروس كورونا.

Bad News! Four difficult months to come regarding the COVID-19 pandemic. About 90% of the world population inhabit the northern hemisphere, where cold weather kicked in. Influenza coinciding with Coronavirus will create havoc around the world. Therefore, taking the influenza vaccine, hand washing, wearing masks, and social distancing are priorities this season. Such practices will help reduce the spread of both viruses.

COVID-19 virus has not mutated significantly to affect its biological activity. So far, only a few minor changes have occurred since its emergence, and any vaccine developed today will be effective against all strains infecting humans.

Pfizer vaccine
is a success. However, it is worth knowing that this type of (mRNA) vaccine has never been licensed before for human use. Moreover, storing the vaccine requires an ultra-cool temperature (-80

C), which many low-income countries could struggle to meet. Additionally, pregnant women, people with chronic allergies, and humans under 18 are all excluded from vaccination. How long the acquired immunity will last remains to be answered.

Could exposure to many microbes protect us against various diseases including the COVID-19? The human body harbors more than 1 kg of microbes (microbiome), mainly in the digestive system. This microbiome helps in the digestion of foods, protects against disease-causing bacteria, tunes the immune system, and produces vitamins that we may benefit from absorbing.
هل تعرضنا للكثير من الجراثيم يحمينا من عدة أمراض ومنها الكورونا؟ جسم الإنسان يحتوي على ما يزيد عن 1 kg من الجراثيم (ميكروبيوم) ومعظمها موجود قي الجهاز الهضمي . وجود هذه الجراثيم يساعد على هضم الأكل, يحمي من الجراثيم الممرضة, يضبط ايقاع جهاز المناعة, ويفرز فيتامينات قد يستفيد الجسم منها بإمتصاصها .
Mutation in viruses is not an unusual phenomenon. They either mutate to become attenuated, more transmissible or become more virulent. Such strategy helps viruses to maintain continuity. Sometimes, infected patients may have several mutants inside their bodies.
إن الطفرات في الفيروسات ليست ظاهرة غير عادية. إما أنها تتحول لتصبح ضعيفة ، أو أكثر قابلية للانتقال أو تصبح أكثر ضراوة. تساعد هذه الاستراتيجية الفيروسات في الحفاظ على الاستمرارية. في بعض الأحيان ، قد يكون لدى المرضى المصابين عدة طفرات داخل أجسامهم.
Mutants are detected by sequencing the virus RNA which, in the case of Covid-19 virus, is made up of 30,000 letters in a particular order and by comparing it to the sequence of the original reference Wuhan strain. Any change in the order of these letters is considered a mutant.

I expect we will see many Covid-19 virus mutants in the future. This is mainly due to several stresses on the virus including its transmissibility between large numbers of people, mass vaccination and treatments.

RNA viruses are unstable and tend to mutate very fast. However, COVID-19 virus, which is a RNA virus, possesses enzymes that proofread it’s genetic sequence while multiplying and tend to mutate slower than other RNA viruses. It mutates around once or twice a month.

Possibly accumulation of mutations leads to major mutants such as the ones currently emerging in England and South Africa and others.

So far, there are tens of thousands of Covid-19 virus minor mutants. The latter become of concern if the mutations lead to major changes in the viral structural parts upon which a vaccine is designed. This may render vaccines ineffective.

It is very important to note that the origin of the virus has not yet been determined and now, it is going to be virtually impossible to find out. Therefore, if we eradicate it from the community it may re-start again as it did first time.

The only virus that was successfully eradicated, so far, is the smallpox virus. Poliovirus is virtually eradicated; it remains infecting very few cases in remote areas in poor countries.

Some Covid-19 vaccines are based on injecting a genetic sequence piece of the messenger RNA (mRNA) coding for the major surface protein (Spike protein) (S-protein) of the virus. The message gets into the human cell programmes it to synthesise the protein which in turn induces immune response and memory. Subsequent exposure to the wild virus, the immune system remembers prior exposure to the surface protein and prevents it from binding to human cell surface.

The virus is made up of 29 proteins and only the surface S-protein has been used in most of the vaccines developed. The role of the remaining 28 proteins and their role, if any, in immunity remains to be determined.

Many vaccines employed today have never been used in humans. They have been tested on enough and approved for emergency use. This means the companies are not held legally responsible for any future side effects of these vaccines. However, monitoring the efficacy and safety of these vaccine will be followed up for at least 2 years.

Depending on their efficacy, most of the vaccines protect individuals from developing serious clinical symptoms of Covid-19 disease; they do not prevent you from catching the virus. So, you may contract the virus, but, if it is transmitted to others remains to be answered. On the other hand, Oxford/Astrazenca vaccine may protect (62% to 90%) humans not only from developing serious covid-19 disease but also protects about 25% of volunteers from acquiring the virus. Oxford team performed PCR on volunteers weekly whereas Moderna and BioNTech/Pfizer did not. They waited until volunteers had symptoms of covid-19 disease.

Pregnant and breastfeeding women and those suffering from severe allergy to medicines or foods are advised not to be vaccinated. All the vaccines have not yet been tested on population aged less than 18 years old.

Currently emerging data show that the first dose of some vaccines seems to provide significantly enough immunity acquired to prevent serious illness symptoms. This seems to support British vaccination campaign plan. I think It is wise for other countries to follow suit. this would provide some form of immunity and give the health authorities to cope with the disease.

Today, February 28, 2021 the FDA approved the single-shot Johnson & Johnson Covid-19 vaccine. It is stored in refrigerator. Is 85% effective at preventing serious illness. The company will provide COVAX scheme with 500 million doses for poor countries. Other countries also ordered jabs: UK-30 million; EU-200 million, Canada- 38 million.

Everybody is asking do I still catch the coronavirus to if I took the vaccine? Well! Pfizer/BioNTech vaccine gives you 75% chance of NOT getting the virus 12 days after the first dose.

I call on specialists and non-specialists to stop frightening people from the vaccines. The vaccines are safe and effective and were developed hastily. If these vaccines were not safe, world countries would not administer it to the medical staff in the first instance. (video link)
أدعو المتخصصين وغير المتخصصين إلى التوقف عن تخويف الناس من اللقاحات ضد مرض كوفيد -19. اللقاحات آمنة وفعالة ولم يتم تطويرها على عجل ولو كانت هذه اللقاحات خطيرة لما أُعطيت وستعطى أولاً للجهاز الطبي في العديد من البلدان.
Why health authorities in Lebanon don’t carry out a comprehensive survey to determine the percentage of people who are positive for COVIDー19 antibodies? This would tell us how far we from herd are immunity and help us open economy as early as possible.

If the most common coronavirus variant in Lebanon is highly transmissible (like UK B.1.1.7), we may need to vaccinate more than 90% of people to achieve herd immunity. So, we need to plan ahead number of doses needed and to sequence as much genomes as possible.

It may take us more than two years to completely control the COVID-19 virus. This is due many reasons. One, the worryingly slow pace at which the vaccination campaign is going. Two, lack of appropriate infrastructure in less developed countries to maintain the efficacy of the vaccines. Three, lack of financial support to purchase enough doses to establish herd immunity (ca 80% of population) in these countries. Eventually, we will end the pandemic, but, will not be able to eradicate the virus. No one is going to be safe until everybody on this planet is safe.

The best way to control an infectious agent is to control it at its source. By 15th February 2021, the World Organisation for Animal Health (OIE) tracked 458 outbreaks of SARS-CoV-2 in at least 8 animal species, including wild counterparts; these were identified by the presence of the virus RNA. Therefore, knowing the origin of COVID-19 virus is virtually impossible to determine. So, at the moment, there is no way the virus will disappear. Will it re-emerge again in different way?

Unlike many countries, Japan requires clinical trials for new drugs/vaccines be conducted with Japanese patients prior approval. So far, only Pfizer/BioNTech COVID19 #vaccine is approved & Oxford/AstraZeneca's awaits approval. This is slowing down the vaccination pace.

Scientist just now are trying to develop a universal vaccine against human coronaviruses. A universal vaccine means to find a conserved part in the spike protein which does not mutate and then use it as a universal vaccine. The irony is that this part of coronaviruses was designed by two Bangladeshi scientists from Dhaka University in 2014. No one paid attention to this study and only now world scientists are busy developing it.

Preliminary results show efficacy of the Oxford/AstraZeneca vaccine against the South African coronavirus strain. The official report will be published later. A previously published report stated that this vaccine was ineffective against this mutant. This strain is considered one of the most important strains that currently exist due to its rapid spread and high avoidance from some vaccines.

No disease in the history of science had so much attention like coronavirus. Almost everybody living on this planet knows about COVID-19. Emerging data show that the first dose of some vaccines seems to provide enough immunity acquired to prevent serious illness from COVID19. This supports British vaccination campaign plan. I think it is wise in Lebanon to follow suit. It provides some form of immunity and give the health carers to cope patients and the economy may end lockdown early.
